Steps to Create and Embed a PAR-Q
If you require clients to complete a PAR-Q before participating in classes or training, you can integrate a form using the Digital Document system using these steps:
Create a PAR-Q Form
Using Google Forms:
Include fields for client responses, such as name and yes/no questions.
Adjust Permissions: Set the form’s permissions to Share (not private) so anyone with the link can access it.
Get the Embed Code:
In Google Forms, go to Settings > Embed Code (look for the <> icon).
Copy the embed code (not the URL link).
Note: Make sure to copy the embed code and not the URL for the document.
Embedding the PAR-Q into your Digital Document
Log into PushPress: Access your PushPress Control Panel.
From the Apps Menu, select Digital Documents.
Create a new document or edit an existing one.
Embed the Par-Q Form:
Paste the embed code directly into the document.
Add Additional Features:
Use the Digital Document tools to include any necessary “I Agree” checkboxes for client acknowledgment.
Publish the Document to "person" to ensure everybody completes the PAR-Q.
Important Notes
Ensure the Google Form’s sharing settings allow access for all users.
Clients can complete the Par-Q directly within the Digital Document interface.
Adding “I Agree” checkboxes ensures acknowledgment of any additional terms or conditions.
How to Access and Manage Completed PAR-Q Forms
By accessing the Responses tab and using the Individual view, you can efficiently review each submission and ensure all necessary information has been completed.
Log into Google Forms:
Use the same Google account you used to create the PAR-Q form.
Open the PAR-Q Form
Click the Responses tab at the top of the form page.
Choose a Viewing Option, You have three options to view responses:
Summary: Provides an overview of all responses.
Question: Groups responses by each question.
Individual: Displays each completed form individually (recommended for reviewing specific client submissions).
