Overview of the PushPress Lead Capture Form — what it looks like, how edit it, to share it, and what happens when a prospect submits it.

Overview

The Lead Capture Form is a public-facing page your gym shares to collect prospect information. When someone submits it, they're automatically added as a lead in your Core account and your staff receives a notification.

The form URL format is: yourgym.pushpress.com/open/interested


What the Form Looks Like

The form displays your gym's branding — your primary color in the header and your gym name — so prospects get a consistent experience. The form is fully responsive and works on mobile.

Always-visible fields: First name, Last name, Email.

Configurable fields (shown or hidden based on your Lead Capture Settings): Date of birth, Phone, Postal code, Lead type, Objectives, Referral source.


Sharing the Form

The form URL can be shared anywhere:

  • Linked from your website

  • Pasted in a social media bio or post

  • Included in email campaigns

  • Added as a QR code for in-gym signage

The URL does not change when you update the form settings — any existing links will reflect your current configuration automatically.


What Happens After Submission

  1. The prospect sees a confirmation message on screen.

  2. A lead profile is created in your Core account with status Lead.

  3. All admins receive a new lead email notification.

  4. If you have PushPress Grow, the lead syncs and can trigger automated follow-up workflows.


Configuring the Form

All form settings are managed under Settings → Lead Capture (or navigate to /settings/leads).

Configuring Form Fields

Each field can be set to one of three states:

  • Hidden — Field is not shown on the form.

  • Optional — Field is shown but not required to submit.

  • Required — Field must be filled in before the form can be submitted.

Configurable fields: Date of birth, Phone, Postal code, Lead type, Objectives, Referral source.

First name, last name, and email are always shown and always required.


Title and Description

Customize the heading and introductory text shown at the top of the form:

  • Form Title — Displayed prominently at the top. Default is your gym name.

  • Form Description — Optional supporting text shown below the title.


Lead Types and Objectives

If the Lead Type field is enabled, prospects can self-select what they're interested in (e.g. Group Classes, Personal Training, Open Gym). Configure available options in the Lead Types section.

If the Objectives field is enabled, prospects can describe their goals. This field is free text.

Note: The Objectives field is shown on the form. The response is saved to the lead's profile and will be visible in an upcoming update.


Previewing the Form

The preview panel on the right side of the settings page shows the live form exactly as prospects will see it. Toggle between Desktop and Mobile views to check the layout at both sizes before saving.

Saving Changes

Click Save to apply your settings. Changes take effect immediately on the public form.
