How to Use the Waiver Form in PushPress Grow

The Waiver form lets your gym collect signed liability agreements from leads and members before they attend their first class — automatically, with no manual follow-up.

Why Use the Waiver Form?

  • No manual work: The form is sent automatically and your team is notified when it's completed.

  • Stay protected: Make sure every lead or member has agreed to your terms before their first session.

  • Flexible setup: Use it with the free trial snapshot or drop it into any existing workflow.


What Comes With the Waiver Snapshot

When you download the Waiver from the Content Library, you get two things:

  1. A Waiver Survey — a digital form your leads or members complete and submit

  2. A Waiver Submission Workflow — an automation that runs when the form is submitted

Here's what the workflow does automatically:

  • Sends your team a notification that the Waiver has been completed

  • Adds a tag to the contact confirming their Waiver is on file


Important: Customize Your Waiver Before Using It

The Waiver that comes in the snapshot is a short, general liability form. It is a starting point — not a finished legal document.

Important: Before sending this Waiver to anyone, consult your legal team or legal representation. Make sure the content is compliant for your gym and covers everything you need. Add, edit, or remove any language as required.

To edit the Waiver:

  1. Go to WebsitesFormsSurveys

  2. Open the Waiver survey

  3. Click into any field and type or paste your updated content

  4. Save your changes


How to Find the Waiver Form

  1. In PushPress Grow, click Websites in the left menu

  2. Go to Forms

  3. Click the Surveys tab

  4. Find and open the Waiver survey

Note: The Waiver is built as a survey so contacts move through it one step at a time — it creates a smoother experience than a standard form.


How to Share the Waiver With Contacts

You have a few options:

Option 1: Use the Free Trial Snapshot (Recommended)

If you've downloaded the Free Trial Class or Free Trial Appointment Calendar snapshot, the Waiver is already built in. When a lead books a trial class or appointment, the Waiver is sent automatically as a follow-up — no extra setup needed.

Option 2: Add the Waiver to Any Existing Workflow

  1. Go to WebsitesFormsSurveys

  2. Open the Waiver survey

  3. Copy the Share Link

  4. Paste that link into any email, SMS, or workflow step in Grow — such as after an appointment booking or free trial class confirmation

Option 3: Send It as a Text Message Snippet

You can save the Waiver link as a snippet and send it automatically through text message. This is a quick way to get the Waiver out without building a full workflow.

Note: If you're using the Waiver snapshot on its own (without the free trial snapshot), you'll need to decide how and when to send the link to your contacts. The snapshot itself only handles the notification and tag — it does not automatically send the form.


Frequently Asked Questions

Where do I find the Waiver in Grow? Go to WebsitesFormsSurveys. The Waiver survey will be listed there.

How does my team get notified when someone submits? The Waiver Submission Workflow sends your team a notification as soon as the form is completed.

Does the Waiver work with my free trial booking flow? Yes. If you've downloaded the Free Trial Class or Free Trial Appointment snapshot, the Waiver is already wired in and sends automatically when someone books.

What if I don't use the free trial snapshot? Grab the share link from the Waiver survey and paste it into any email, SMS, workflow, or text message snippet in your account.

Can I edit the Waiver content? Yes. Click into the survey and type or paste whatever language you need. Always run your final version by your legal team before sending it to contacts.

What tag gets added when someone completes the Waiver? A tag is added to the contact's record confirming their Waiver is complete. You can use this tag to filter contacts or trigger other automations.

Is this Waiver legally compliant? The form is a general liability starting point. It is not legal advice. Consult your legal representation to make sure the content meets your gym's needs and local requirements.


Troubleshooting

If the Waiver notification is not coming through, check:

  • Is the Waiver Submission Workflow turned on?

  • Is the contact's email or phone number on file in Grow?

If the Waiver is not being sent automatically, check:

  • Are you using the Free Trial snapshot? If not, you'll need to add the share link to your own workflow or send it manually.

Important: The Waiver does not automatically block a contact from booking or attending. It collects agreement information for your team to track. Always confirm a Waiver is on file before a contact's first session.
