How to Use the PAR-Q Form in PushPress Grow

The PAR-Q form lets your gym collect health screening information from leads and members before they attend their first class - automatically, with no manual follow-up.

Why Use the PAR-Q Form?

  • No manual work: The form is sent automatically and responses are emailed to your team.

  • Stay informed: Get notified right away when someone submits their PAR-Q.

  • Flexible setup: Use it with the free trial snapshot or drop it into any existing workflow.


What Is a PAR-Q?

A PAR-Q (Physical Activity Readiness Questionnaire) is a short health screening form. It asks leads or members basic health questions before they start exercising. It helps your team know if anyone needs follow-up before their first session.

Note: The PAR-Q is for health screening only. Waivers and medical clearance forms are handled separately.


What Comes With the PAR-Q Snapshot

When you receive the PAR-Q snapshot in your account, you get two things:

  1. A PAR-Q Survey — a digital form your leads or members fill out

  2. A PAR-Q Submission Workflow — an automation that runs when the form is submitted

Here's what the workflow does automatically:

  • Adds a tag to the contact confirming their PAR-Q is complete

  • Sends your team an internal notification with an email showing all of their responses


How to Find the PAR-Q Form

  1. In PushPress Grow, click Websites in the left menu

  2. Go to Forms

  3. Click the Surveys tab

  4. Find and open the PAR-Q survey

Note: The PAR-Q is built as a survey (not a form) so contacts can move through it one question at a time — similar to a guided questionnaire flow.


How to Share the PAR-Q With Contacts

You have two options:

Option 1: Use the Free Trial Snapshot (Recommended)

If you've downloaded the Free Trial Class or Free Trial Appointment Calendar snapshot, the PAR-Q is already built in. When a lead books a trial class or appointment, the PAR-Q is automatically sent as a follow-up — no extra setup needed.

Option 2: Add the PAR-Q to Any Existing Workflow

If you're not using the free trial snapshot, you can add the PAR-Q link to any workflow, email, or SMS manually.

  1. Go to WebsitesFormsSurveys

  2. Open the PAR-Q survey

  3. Click Integrate and copy the Share Link

  4. Paste that link into any email, SMS, or workflow step where you want the form to appear


How to Customize the PAR-Q Questions

You can add or edit questions in the PAR-Q survey. If you do, there's one extra step.

Important: If you add, edit, or delete any PAR-Q questions, you must also update the email inside the PAR-Q Submission Workflow. The new fields will not automatically appear in your team's notification email.

To update the notification email after changing a question:

  1. Open the PAR-Q Submission Workflow

  2. Find the email notification step

  3. Click into the email body and place your cursor where you want the new field to appear

  4. Click the tag icon and go to ContactCustom Fields

  5. Find the new or updated field and click to insert it

  6. Save the workflow


Frequently Asked Questions

Where do I find the PAR-Q form in Grow? Go to WebsitesFormsSurveys. The PAR-Q survey will be listed there.

How does my team get notified when someone submits? The PAR-Q Submission Workflow sends an internal notification email to your team with all of the contact's responses included.

Does the PAR-Q work with my free trial booking flow? Yes. If you've downloaded the Free Trial Class or Free Trial Appointment snapshot, the PAR-Q is already wired in. It sends automatically when someone books a trial.

What if I don't use the free trial snapshot? No problem. Grab the share link from the PAR-Q survey and paste it into any email, SMS, or workflow step in your account.

Can I add my own questions to the PAR-Q? Yes, but if you add or remove questions, you need to update the notification email inside the PAR-Q Submission Workflow to include the new fields. Otherwise, those responses won't show up in your team's email.

What tag gets added when someone completes the PAR-Q? A tag is added to the contact's record confirming their PAR-Q is complete. This makes it easy to filter or trigger other automations based on completion status.


Troubleshooting

If the PAR-Q notification email is missing responses, check:

  • Did you recently add or change a question in the survey?

  • If yes, update the email in the PAR-Q Submission Workflow to include the new custom fields

If the PAR-Q is not being sent automatically, check:

  • Are you using the Free Trial snapshot? If not, you'll need to add the share link to your own workflow manually

  • Is the PAR-Q Submission Workflow turned on?
