Setup
Begin with setting up the plan. Set up the plan as you would any other plan. When setting up any check-in restrictions, make those settings based off the basic membership plan, and NOT on PT restrictions.

Selling and Scheduling
Sell the plan as normal. You can do that either through the control panel or via landing page.
Now you need to add appointment credits to the member's profile. Since the member already paid for the plan and the sessions, you'll need to add the credits and COMP the credits. This would ensure the member does not get billed for the credits, but preserves the appointment rate, for payroll purposes.
From there, you can schedule and check the member into their appointment like normal.
